Mecasermin is a somatomedin C or human insulin-like growth factor-1 (rhIGF-1) produced by recombinant DNA technology. IGF-1 consists of a chain of 70 amino acids, with three intramolecular disulfide bonds and a molecular weight of 7,649 Da. The amino acid sequence of this product is identical to that of endogenous human IGF-1. The rhIGF-1 protein is synthesized by the E. coli bacteria previously modified by introduction of the gene encoding human IGF-1.

IGF-1 is the main hormonal mediator of stature growth. Under normal circumstances, growth hormone (GH) normally binds to its receptor in the liver and other tissues and stimulates the synthesis/secretion of IGF-1. In target tissues, the IGF-1 type 1 receptor, similar to the insulin receptor, is activated by IGF-1, triggering the transmission of intracellular signals stimulating multiple processes causing of stature growth. The metabolic effects of IGF-1 include stimulating the absorption of glucose, fatty acids and amino acids allowing metabolism to support tissue growth.
